ASCVD Risk Calculator Explained
Understanding your 10-year risk of heart attack and stroke.
The ASCVD Risk Estimator Plus is the standard tool used by clinicians to estimate a person's 10-year risk of atherosclerotic cardiovascular disease—meaning heart attack, stroke, or cardiovascular death. It is based on the Pooled Cohort Equations developed by the American College of Cardiology and American Heart Association.

- Age (40–79 years)
- Sex
- Race (Black or non-Black)
- Total cholesterol and HDL cholesterol
- Systolic blood pressure
- Whether you are on blood pressure medication
- Diabetes status
- Smoking status
Interpreting Your 10-Year Risk

|---|---|
| < 5% | Low risk |
| 5% to < 7.5% | Borderline risk |
| 7.5% to < 20% | Intermediate risk |
| ≥ 20% | High risk |
What the Score Means for Treatment
Your ASCVD risk helps determine whether lifestyle changes alone are enough or if statin therapy should be considered. People with diabetes, very high LDL cholesterol, or a prior cardiovascular event often fall into a "risk-enhanced" category regardless of the calculated score.
Limitations
The calculator was developed in specific U.S. populations and may underestimate or overestimate risk in some ethnic groups, very fit individuals, or those with strong family history. It should be used as a conversation starter with your clinician, not a definitive verdict.
