Warfarin dosing adjustments are based on current INR and target range (usually 2.0–3.0). Follow institutional protocols and recheck INR per guidance.

  • <2.0 (Subtherapeutic): Increase weekly dose by 0–15% depending on how low the INR is.
  • 2.0–3.0 (Therapeutic): Continue current dose. Recheck per protocol.
  • 3.1–4.0 (Supratherapeutic): Decrease dose or hold. Recheck in 1–3 days if >4.0.
  • >4.0: Hold dose. Consider low-dose vitamin K if bleeding. Recheck in 24h.
