BISAP Score (Bedside Index of Severity in Acute Pancreatitis)

Predicts severity and mortality in acute pancreatitis using 5 bedside criteria assessable within 24 hours of admission.

Blood urea nitrogen > 25 mg/dL (8.92 mmol/L)

Altered mental status (GCS < 15)

Systemic inflammatory response syndrome (>=2 criteria)

Age greater than 60 years at presentation

Pleural effusion on imaging (CXR, US, CT)

Clinical Notes & Warnings

  • Superior to Ranson criteria for early prediction (available at 24h vs 48h).
  • Does not require CT imaging for severity assessment.
  • SIRS criteria: Temp >38 or <36 C, HR >90, RR >20 or PaCO2 <32, WBC >12k or <4k or >10% bands.
