BISAP Score (Bedside Index of Severity in Acute Pancreatitis)

Predicts severity and mortality in acute pancreatitis using 5 bedside criteria assessable within 24 hours of admission.

BUN > 25 mg/dL

Blood urea nitrogen > 25 mg/dL (8.92 mmol/L)

Impaired Mental Status

Altered mental status (GCS < 15)

SIRS Criteria >= 2

Systemic inflammatory response syndrome (>=2 criteria)

Age > 60 years

Age greater than 60 years at presentation

Pleural Effusion

Pleural effusion on imaging (CXR, US, CT)

Superior to Ranson criteria for early prediction (available at 24h vs 48h).
Does not require CT imaging for severity assessment.
SIRS criteria: Temp >38 or <36 C, HR >90, RR >20 or PaCO2 <32, WBC >12k or <4k or >10% bands.
