Clinical Notes & Warnings
- ADA recommends target 140-180 mg/dL for most ICU patients.
- Tight glycemic control (80-110 mg/dL) increases hypoglycemia risk without mortality benefit.
- Do not exceed 10 units/hr without physician review.
- Stop or reduce infusion if BG < 100 mg/dL to prevent hypoglycemia.
- This is a general guideline; institutional protocols may differ.
Common ICU protocols: Yale (0.02-0.06 units/kg/hr with BG-based titration), Leuven (tight 80-110). NICE-SUGAR showed harm from tight control.
