Note

CDC updated conversion factors in 2022. The 2016 CDC guideline used different factors (e.g., hydrocodone = 1, tramadol = 0.1 vs older 0.2). The conversion table above reflects the 2022 updated values.

Warnings & Limitations

  • Do NOT use for converting FROM methadone or buprenorphine to other opioids.
  • Cross-tolerance incomplete: reduce converted dose by 25-50% when switching opioids.
  • Fentanyl patch conversion is approximate; individual response varies.
  • Partial agonists (buprenorphine) have ceiling effects not captured by MME.
  • Benzodiazepines, alcohol, and sleep apnea increase overdose risk independent of MME.
  • MME does not reflect pharmacodynamic differences between opioids.
  • For methadone: consult specialist/pain/palliative care before any conversion.
