Includes myocardial infarction, positive stress test, or use of nitroglycerin/typical angina.
Prior heart failure or pulmonary edema.
Prior stroke or transient ischemic attack.
Diabetes requiring insulin therapy.
Preoperative creatinine >2.0 mg/dL (176.8 µmol/L).
Suprainguinal vascular, intraperitoneal, intrathoracic surgery.
