Generalized Anxiety Disorder 7-item scale (GAD-7)
A 7-item self-report questionnaire for screening and severity assessment of generalized anxiety disorder. Each item is scored 0-3 based on frequency over the past 2 weeks.
Note: A cut-off of ≥10 is commonly used in primary care to flag clinically significant anxiety. A 2-item version (GAD-2) exists for rapid screening.

| ● Severe anxiety | 15-21 | Severe anxiety; active treatment strongly recommended. |
