CKD-EPI 2021 vs Cockcroft-Gault: Which Calculator Should You Use?
Two formulas, two different purposes. Here is how clinicians and patients should interpret them.
If your blood test shows elevated creatinine, your doctor may quote two numbers: eGFR and creatinine clearance (CrCl). They are not the same thing. Choosing the right formula depends on why you are calculating kidney function in the first place.

CKD-EPI 2021: The Standard for Diagnosis
The Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) 2021 equation is the current gold standard for detecting and staging chronic kidney disease (CKD). It replaced the older MDRD equation and—importantly—removed the race-based coefficient that previously disadvantaged Black patients.
Best used for:
- CKD screening and staging
- Monitoring kidney function over time
- Population health and research
Limitations: CKD-EPI normalizes results to a body surface area of 1.73 m². That means it may underestimate kidney function in very muscular or obese individuals.
Cockcroft-Gault: The Standard for Drug Dosing
The Cockcroft-Gault formula has been around since 1976. Unlike CKD-EPI, it does not adjust for body surface area. Instead, it estimates absolute creatinine clearance based on age, sex, weight, and serum creatinine.
Best used for:
- Medication dosing adjustments (antibiotics, chemotherapy, contrast dye protocols)
- Pharmacokinetic calculations
Limitations: Cockcroft-Gault assumes ideal body weight. In obesity, it can overestimate kidney function if actual body weight is used, or underestimate it if ideal body weight is too low.

So Which One Should You Use?
If you are a patient trying to understand whether you have kidney disease, focus on the CKD-EPI eGFR. If you are a clinician calculating antibiotic or chemotherapy doses, use Cockcroft-Gault unless your hospital protocol specifies otherwise.
