HAS-BLED Score (Bleeding Risk in Atrial Fibrillation)

Estimates one-year major bleeding risk in patients with atrial fibrillation on anticoagulation. Used alongside CHADS2-VASc to guide anticoagulation decisions.

Uncontrolled hypertension with systolic BP > 160 mmHg

Dialysis, renal transplant, or serum creatinine >= 200 micromol/L (>=2.3 mg/dL)

Chronic liver disease (cirrhosis) or bilirubin >2x ULN with AST/ALT/ALP >3x ULN

History of stroke

Prior major bleeding, anemia, or bleeding diathesis

Time in therapeutic range < 60% (for patients on warfarin)

Aspirin, clopidogrel, NSAIDs that increase bleeding risk
