Reticulocyte Production Index (RPI)
Adjusts the reticulocyte percentage for anemia severity and bone marrow maturation time to assess effective erythropoiesis.
Warnings:
- RPI requires accurate reticulocyte count and hematocrit measurement
- Normal Hct varies by sex (men ~45%, women ~40%); use appropriate reference
- Reticulocyte count may be falsely elevated in presence of schistocytes or nucleated RBCs
- Maturation time table is an approximation; actual maturation varies
- RPI should be interpreted alongside MCV, iron studies, and peripheral smear
- Some sources use maturation time of 2.5 days for Hct <15%, others use 3.0 days
Note: Reticulocyte Index (RI) = Retic % x (Patient Hct / Normal Hct). RPI = RI / Maturation Time. Normal RPI in the face of anemia indicates production problem. Elevated RPI with anemia indicates blood loss or hemolysis.
Percentage of reticulocytes from complete blood count
Patient's measured hematocrit (or convert from hemoglobin: Hct ~ Hb x 3)
Normal hematocrit for patient population. Default = 45% for men, 40% for women.
