  • A 10-year ASCVD risk estimate for eligible adults aged 30–79 years
  • A 30-year ASCVD risk estimate for eligible adults aged 30–59 years
  • Race-free risk estimation
  • Integration of cardiovascular, kidney, and metabolic health variables

The guideline applies PREVENT-ASCVD particularly to primary-prevention adults without known ASCVD or subclinical atherosclerosis and with an LDL cholesterol level of 70–189 mg/dL.

Pooled Cohort Equations

The ACC/AHA Pooled Cohort Equations were introduced in 2013 and remain embedded in many clinical systems and online ASCVD calculators. They estimate 10-year risk in adults aged 40–79 years using age, sex, race category, cholesterol, blood pressure, diabetes, smoking, and blood-pressure treatment.

Because clinical recommendations and calculator integration are changing, check which equation your calculator uses before interpreting its categories. A percentage produced by PREVENT should not automatically be interpreted using the older Pooled Cohort Equation thresholds.

Information Needed to Calculate ASCVD Risk

The exact inputs vary by calculator, but you should usually have the following information available:

  • Age
  • Sex
  • Total cholesterol
  • HDL cholesterol
  • Systolic blood pressure
  • Whether blood pressure is being treated
  • Current smoking status

Use accurate and current measurements

Risk estimates are only as reliable as the information entered. Use a recent lipid profile and a representative blood-pressure reading. A single blood-pressure measurement taken during pain, anxiety, acute illness, or immediately after physical exertion may not reflect usual blood pressure.

Smoking status should reflect current combustible tobacco use according to the calculator’s definition. Diabetes should not be selected merely because of one mildly elevated glucose measurement; use an established diagnosis or appropriate diagnostic assessment.

How to Calculate ASCVD Risk Step by Step

  1. Confirm that the calculator is appropriate. Determine whether the person is within the calculator’s validated age range and does not already have established ASCVD.
  2. Identify the equation being used. Look for PREVENT-ASCVD or Pooled Cohort Equations in the calculator description.
  3. Collect current clinical data. Obtain cholesterol results, blood pressure, smoking status, diabetes history, kidney-function information, and relevant medication use.
  4. Enter the information carefully. Confirm that cholesterol and blood-pressure units are correct. Do not confuse total cholesterol with LDL or HDL cholesterol.
  5. Record the estimated percentage and time horizon. For example, document whether the result is a 10-year ASCVD risk or a 30-year risk.

How to Interpret PREVENT-ASCVD Results

The 2026 ACC/AHA dyslipidemia guideline uses the following 10-year PREVENT-ASCVD categories:

The 2026 guideline notes that statin therapy may be reasonable in selected adults with a 10-year risk below 3% when LDL cholesterol is 160–189 mg/dL or the 30-year ASCVD risk is at least 10%.

How Older Pooled Cohort Equation Results Were Interpreted

Many existing articles and calculators still display the categories used with the Pooled Cohort Equations:

Consider a hypothetical 55-year-old adult without known cardiovascular disease. The person has a total cholesterol of 220 mg/dL, HDL cholesterol of 42 mg/dL, systolic blood pressure of 142 mmHg, is receiving antihypertensive treatment, currently smokes, and does not have diabetes.

After entering these values, the calculator may return a 10-year percentage. The exact result depends on the equation and any additional variables included. Rather than focusing only on the number, the interpretation should identify the major modifiable contributors:

  • Current smoking
  • Elevated systolic blood pressure
  • Unfavorable cholesterol levels

Smoking cessation and better blood-pressure control may meaningfully reduce risk. Lipid-lowering therapy may also be appropriate, depending on the calculated category, LDL level, additional risk factors, potential contraindications, and the person’s informed preferences.

  • Elevated lipoprotein(a)
  • Elevated apolipoprotein B
  • Persistently elevated triglycerides
  • Evidence of subclinical atherosclerosis

The relevance of each factor depends on the individual situation. Their presence does not automatically require medication, but they may shift the balance toward more intensive prevention.

When Coronary Artery Calcium Scoring May Help

A coronary artery calcium, or CAC, scan is a noncontrast CT examination that detects calcified plaque in the coronary arteries. It may be considered when the decision about lipid-lowering therapy remains uncertain after calculation and clinical assessment.

A higher CAC score supports the presence of coronary atherosclerosis and may favor treatment intensification. A CAC score of zero can sometimes support postponing medication in carefully selected people, but it does not eliminate risk. Smoking, diabetes, a strong family history, severe hypercholesterolemia, and other high-risk conditions may still justify treatment despite a score of zero.

CAC testing involves radiation exposure, cost, and the possibility of incidental findings. It should therefore be ordered when the result is likely to change management rather than as routine screening for everyone.

  • Established ASCVD: Management follows secondary-prevention guidance rather than a primary-prevention score.
  • LDL cholesterol of 190 mg/dL or higher: Treatment is generally considered regardless of calculated short-term risk because this level suggests substantial lifelong exposure and possible familial hypercholesterolemia.
  • Pregnancy: Standard risk estimates are not designed to determine medication decisions during pregnancy.
  • Age outside the validated range: The calculator may not provide a reliable estimate.
  • Non-US populations: Risk equations developed in US cohorts may overestimate or underestimate risk in other populations.
  • Major chronic illness: Conditions such as severe kidney disease, inflammatory disease, cancer therapy exposure, or limited life expectancy require individualized assessment.

The next step is to identify which modifiable factors are contributing to risk. A prevention plan may include:

  • Stopping smoking and avoiding tobacco exposure
  • Treating hypertension
  • Improving dietary quality
  • Performing regular aerobic and resistance activity when medically appropriate
  • Managing diabetes and chronic kidney disease
  • Achieving or maintaining a healthy body composition
  • Discussing lipid-lowering therapy when indicated
  • Monitoring cholesterol, blood pressure, kidney function, and treatment response

Aspirin should not be started solely because an ASCVD percentage is elevated. Primary-prevention aspirin decisions require a separate assessment of cardiovascular benefit, age, gastrointestinal or intracranial bleeding risk, concurrent medications, and patient preferences.

Limitations of ASCVD Risk Calculators

Risk calculators estimate outcomes in groups of people with similar characteristics; they cannot predict exactly what will happen to one individual. Calibration may vary across countries, ethnic groups, socioeconomic settings, and changing treatment patterns.

Age strongly influences short-term risk. A younger adult with smoking, severe hypertension, and high LDL cholesterol may still receive a relatively low 10-year estimate despite substantial lifetime risk. Conversely, an older adult may receive a high estimate largely because of age, even when several modifiable factors are well controlled.

The result should therefore be viewed as the beginning of a structured prevention discussion—not the final clinical decision.
