⚠️ Warnings & Contraindications

  • Higher incidence of GI side effects (nausea, vomiting, diarrhea, abdominal pain) than azithromycin/clarithromycin
  • Risk of infantile hypertrophic pyloric stenosis (IHPS) in infants <6 weeks
  • Hepatotoxicity (cholestatic hepatitis) reported
  • Multiple drug interactions via CYP3A4 inhibition
  • May prolong QT interval
  • Erythromycin estolate associated with higher hepatotoxicity risk

ℹ️ Clinical Notes

Suspension: 200mg/5mL, 400mg/5mL (ethylsuccinate). Tablets: 250 mg, 500 mg. Base, ethylsuccinate, and stearate formulations have different bioavailability. Given 4 times daily, adherence may be poor.
