⚠️ Warnings & Contraindications

  • NEPHROTOXICITY: Monitor renal function (SCr, BUN, urine output) daily
  • OTOTOXICITY: Irreversible vestibular/cochlear damage possible
  • Once-daily dosing (extended interval) may be less toxic than divided dosing
  • Therapeutic drug monitoring is essential
  • Avoid concurrent nephrotoxins (vancomycin, amphotericin, contrast dye)
  • Neonates have immature renal function and require longer intervals
  • Dose reduction in renal impairment essential

ℹ️ Clinical Notes

IV/IM administration only. Once-daily dosing (5-7.5 mg/kg) is preferred in children due to concentration-dependent killing and reduced toxicity. Extended interval TDM: peak 20-30 mcg/mL, undetectable trough at 8-12 hours.
