⚠️ Warnings & Contraindications
- RESPIRATORY DEPRESSION: Monitor respiratory rate and oxygen saturation
- NEONATAL CAUTION: Neonates have reduced clearance and increased sensitivity
- Constipation is universal - start stool softeners prophylactically
- Nausea and vomiting common initially; premedicate with antiemetic if needed
- Physical dependence develops with prolonged use; do not stop abruptly
- Have naloxone available for emergency reversal
- Never give more frequently than ordered - cumulative effect is dangerous

Oral solution: 2 mg/mL, 4 mg/mL, 20 mg/mL. IV: multiple concentrations available. Oral bioavailability approximately 30-40%. PO:IV ratio approximately 3:1 (i.e., oral dose is 3x IV dose).
