0=Nonspecific; 1=Mixed typical/atypical; 2=Typical exertional chest pain with radiation, diaphoresis, nitrate response
0=Normal; 1=LBBB, LVH, nonspecific ST-T without ST deviation; 2=ST elevation/depression not due to LBBB/LVH
Risk factors: hypertension, diabetes, hyperlipidemia, smoking, obesity, family history
